For decades the scale and the BMI chart have been the default way to judge a healthy weight. Both miss the thing that actually matters: where your body stores fat. Fat packed around your organs is far more dangerous than fat on your hips or thighs, and the cheapest, most reliable way to spot it is a tape measure and a little arithmetic. Waist-to-height ratio — your waist divided by your height — is that check, and a growing body of research suggests it predicts health risk better than BMI. This tool computes it and tells you exactly what it means.

What waist-to-height ratio is

The idea is almost too simple. Measure your waist, measure your height in the same units, and divide. A person 70 inches tall with a 35-inch waist has a ratio of 0.50; trim the waist to 33 inches and it falls to 0.47. That single number captures how much fat you carry around your middle relative to your frame, which is what makes it fairer than waist circumference alone — a 32-inch waist means something very different on someone 5'2" than on someone 6'4".

The headline rule, popularized by researcher Margaret Ashwell, is one most people can remember: keep your waist to less than half your height. That's a ratio under 0.5. It's the boundary this calculator uses, and it's backed by the UK's National Institute for Health and Care Excellence (NICE), which in 2022 began recommending waist-to-height ratio as a practical measure of central fat alongside BMI.

Why it beats BMI

BMI — weight divided by height squared — has two well-known flaws. It can't tell muscle from fat, so it wrongly flags lean, muscular people as "overweight." And it ignores fat distribution entirely, so two people with identical BMIs can have very different risks depending on whether their fat sits on their belly or their limbs. Waist-to-height ratio fixes both. It zeroes in on belly fat, and because muscle doesn't pile up around your waistline the way visceral fat does, it isn't fooled by a muscular build.

The evidence backs this up. A 2012 systematic review and meta-analysis concluded that waist-to-height ratio was a significantly better screening tool than BMI for diabetes, high blood pressure, and cardiovascular risk in both men and women. Another large analysis found it discriminated those outcomes more accurately than waist circumference, too. And in a striking long-term study, researchers estimated that a 30-year-old man with a ratio of 0.7 (versus a healthy 0.5) could expect to lose around 7 years of life; for a woman the figure was nearly 5 years.

One more advantage worth its weight in clinical guidelines: the 0.5 boundary works for both sexes and across most ethnic groups — no separate charts required. BMI's healthy ranges, by contrast, shift by ethnicity.

What your number means

The calculator sorts your ratio into four zones, following the NICE and Ashwell thresholds:

  • Below 0.4 — possibly underweight. Not a target to chase; very low values can signal you are carrying too little. Worth a conversation with your doctor if it's unexpected.
  • 0.4 to 0.49 — healthy. No increased risk from central fat. This is the zone to stay in.
  • 0.5 to 0.59 — increased. Central fat is starting to raise your health risk. A good moment to act on diet, movement, and waistline before it climbs further.
  • 0.6 and above — high. Risk is further increased; this is worth taking seriously and raising with a clinician.

Why belly fat is the dangerous kind

Not all fat behaves the same. The subcutaneous fat you can pinch under the skin is far less metabolically harmful. The fat this ratio detects — visceral fat, wrapped around your liver, pancreas, and intestines — is metabolically active and harmful. It pumps out inflammatory signals, drives insulin resistance, and is tightly linked to type 2 diabetes, heart disease, fatty liver, and several cancers. That's why a person who looks only slightly overweight but carries it all around the middle can be at higher risk than someone heavier but more evenly proportioned. Your waist is, in effect, a window onto the fat you can't see.

How to measure your waist correctly

A small measurement error changes your ratio, so technique matters. The most reliable landmark — the one the World Health Organization uses — is the midpoint between the bottom of your lowest rib and the top of your hip bone (iliac crest). That's usually a little above the belly button. Measuring at the navel, the most common DIY method, reads too low in people with more abdominal fat. Here's the reliable way:

  • Measure against bare skin or a thin layer, not over bulky clothing or a belt.
  • Stand up straight, feet slightly apart, weight even, arms relaxed at your sides.
  • Find the midpoint between your lowest rib and the top of your hip bone; wrap the tape around there, keeping it level all the way around (check the back in a mirror).
  • Keep the tape snug but not digging into the skin. Breathe out gently and read the number at the end of a normal exhale — don't suck your stomach in.
  • Measure twice and average. Use the same units for waist and height — both centimetres or both inches.

How this calculator works

There's no hidden model here — that's part of the appeal. The formula is just waist ÷ height, with both measured in the same units, and your result is matched to the four risk bands above. The "healthy waist cap" the tool shows you is simply half your height: the largest waist that keeps you under 0.5. If you're above the line, it tells you roughly how many centimetres or inches would bring you back into the healthy zone — a concrete, trackable goal.

How to bring your ratio down

Visceral fat is stubborn, but it's also the first fat to respond when you make changes — people often lose inches off their waist before the scale moves much. The levers that work:

  • A modest calorie deficit built on whole foods. Cutting refined carbohydrates and sugary drinks while filling up on vegetables, fibre, and protein targets visceral fat especially well.

  • Strength training. More muscle improves insulin sensitivity and makes a deficit easier to sustain.
  • Sleep and stress. Short sleep and chronic stress both raise cortisol, which encourages the body to store fat around the middle. Don't overlook them.
  • Go easy on alcohol. "Beer belly" is more than a saying — alcohol calories preferentially park themselves as visceral fat.

Common questions

How often should I measure?

Monthly is plenty. Waist changes slowly, and measuring the same way each time — same landmark, same time of day — matters more than measuring often.

My BMI is "normal" but my ratio is high. Which do I trust?

This is exactly the case waist-to-height ratio was designed to catch — so-called "normal-weight central obesity." A normal BMI with a high ratio means you're carrying risky visceral fat despite an average weight, and it's worth acting on.

Does the ratio work for older adults?
