OTC vs. Prescription Hearing Aids: Which Should You Buy?

Buy over-the-counter if you are an adult who notices mild to moderate trouble hearing, has no ear symptoms, and is willing to run a setup app. Go through a clinic if your hearing loss is severe, sudden, one-sided, or comes with pain, drainage, or dizziness. That one distinction settles the OTC hearing aids vs prescription hearing aids question for most people.
The rest is money and hand-holding. OTC devices arrive in a box for a few hundred dollars. A prescription pair typically costs thousands and arrives with a professional who measures your hearing, programs the devices to your loss, and fixes them when they are wrong. Both routes can work, but they are not interchangeable, and one is legally off-limits for certain kinds of hearing loss. Below: what the FDA category permits, what a clinic fitting buys, what the research says about the sound difference, and what each route costs. The short answer
OTC is a reasonable first move if most of this describes you:
- You are 18 or older and notice you are missing words, especially in restaurants or on the phone.
- Both ears seem about equally affected, and the change came on gradually over years.
- No ear pain, no drainage, no dizziness, no ringing confined to one ear.
- You are comfortable pairing a device to a phone and running a hearing check in an app.
- You want to spend hundreds, not thousands, before you know whether you will wear them.
A clinic is the better first stop if any of this applies:
- One ear is clearly worse than the other, or the change was sudden.
- You have ear pain, drainage, vertigo, or ringing in only one ear.
- You struggle even in a quiet room with one person talking.
- You have had ear surgery, a perforated eardrum, or repeated ear infections.
- You want someone else to own the problem when the devices sound wrong.
Our view: for a gradual, both-ears loss with no red flags, OTC is a legitimate way to find out whether you will actually wear hearing aids — the question that stops most people long before device quality does. According to the National Institute on Deafness and Other Communication Disorders (NIDCD), only about one in five people who would benefit from a hearing aid uses one.
What the FDA's OTC category actually covers
The FDA issued its final rule creating the over-the-counter hearing aid category on August 17, 2022, and it took effect that October 17. The FDA describes the category as covering air-conduction hearing aids "intended for use by people 18 years of age and older to help with perceived mild to moderate hearing loss," available "without the supervision, involvement, or prescription of a licensed health care professional." The regulatory definition adds that an OTC hearing aid is one that "does not require implantation or other surgical intervention."
Three words in that description do a lot of work:
- Perceived. No audiogram is required. The FDA's rule removed the need for a medical exam, a prescription, or a fitting adjustment by an audiologist.
- Mild to moderate. The FDA states plainly that OTC hearing aids are "NOT for the treatment of severe or profound hearing loss."
- 18 or older. Children are outside the category entirely.
Everything else is a prescription hearing aid. NIDCD describes prescription devices as available "from a hearing health professional who will program them for your degree of hearing loss," and notes they "may be necessary for more significant or complicated hearing loss." That is the whole boundary.
The category belongs to the same shift toward self-managed care that put blood pressure cuffs and other home health monitoring devices in people's kitchens. It also opened a door for sloppy marketing.
"FDA-registered" is not "FDA-cleared" or "FDA-approved"
Any company making medical devices has to register its facility and list its products with the FDA. That is paperwork, not permission. In the FDA's own words, an entry in the registration and listing database "does not denote approval, clearance or authorization of that facility or its medical devices." The agency adds that it does not issue registration certificates at all, and that any representation creating the impression of FDA approval because of registration "is misleading and constitutes misbranding."
A product page shouting "FDA-registered" is telling you the seller filed a form. The FDA says as much on its own OTC hearing aid page: statements such as "FDA Registered" and "FDA Certified" on the labeling "may be misleading." A device that is FDA-cleared or authorized went through a marketing submission the agency actually reviewed.
One nuance before you shop: preset and wireless OTC hearing aids are exempt from FDA premarket review, while a self-fitting OTC hearing aid is a class II device that must clear a 510(k) and meet special controls, including clinical data on its self-fitting strategy (21 CFR 874.3300, 874.3305, and 874.3325). So no "cleared" claim on a simple preset device is not by itself a warning sign — but "FDA-registered" is never evidence that anyone reviewed anything.
What you get from a prescription fitting
The service gap is where the money goes. Four things a clinic sells that a box does not:
A measured audiogram. A professional tests each ear separately across frequencies and charts exactly where and how much you have lost.
Programming to a prescriptive target. Those results feed a validated fitting formula that calculates the gain you need at each pitch instead of applying a generic curve. The American Speech-Language-Hearing Association (ASHA) notes that the most widely used independent prescriptive methods are NAL-NL2 from the National Acoustic Laboratories and Desired Sensation Level (DSL) v5.0.
Real-ear measurement. This is the step most people have never heard of and the one that separates a good fitting from a guess. A thin probe microphone sits in your canal near the eardrum while the hearing aid is in, and the clinician measures the sound actually reaching the eardrum against the target. ASHA calls real-ear measurement "the gold standard in hearing aid fitting verification," and the American Academy of Audiology's best-practice guidance likewise calls for probe-microphone verification. It matters because ear canals differ and because a manufacturer's own starting settings often miss the target: ASHA notes that "for several manufacturers, the actual output of the manufacturer's formula may vary considerably from prescriptive NAL-NL2 targets," and a 2021 study in CoDAS summarizing earlier coupler research reports that a manufacturer's first-fit output has been measured below NAL-NL1 targets by as much as 10 to 15 dB.
Instruction and follow-up. NIDCD describes working through the adjustment period with an audiologist present: practicing insertion and removal, cleaning, changing batteries, adjusting volume, programming the aid for different listening environments, and troubleshooting discomfort and feedback. A clinician may also spot impacted earwax or an asymmetry between ears that an app test cannot see.
Sound quality: is there a real difference?
For mild to moderate hearing loss, the best head-to-head evidence says the gap is smaller than the price difference suggests.
A randomized clinical trial published in JAMA Otolaryngology–Head & Neck Surgery in 2023 assigned 64 adults with mild to moderate bilateral sensorineural hearing loss (mean age 63.6) to either a self-fitting OTC hearing aid or an audiologist-fitted one. The primary outcome was self-reported benefit on the Abbreviated Profile of Hearing Aid Benefit. The self-fitting group showed some early advantages at two weeks, and by the end of the six-week trial the researchers reported that "the differences were not meaningful on any subscale or global benefit scores between the groups." A follow-up analysis in the same journal in 2024 tracked 44 of those participants eight months after fitting and again found "no significant differences" in self-reported benefit.
Take that seriously, but not further than it goes. The authors listed their own limits: blinding was not possible, only one self-fitting device with one fitting method was tested, and the sample was too small for subgroup analysis. Two more things belong on the label: it was run at a university in South Africa, not in a US retail setting, and it was funded in part by hearX, maker of the self-fitting device used in both arms — the paper says the funder supplied devices and software support but otherwise had no role in design, analysis, or publication. Every participant had mild to moderate loss — exactly the group the OTC category was written for. Nothing here says an OTC device will serve someone with severe loss, and there are still few well-controlled studies on the products now for sale. Our read: in the loss range OTC devices are legally sold for, the limiting factor is usually not the amplifier. It is whether the device is set close to what your ears need and whether you wear it daily.
Cost: what each route really runs
Hearing aid pricing is unusually opaque, so treat any single number with suspicion — including these. As of August 2026, the most useful public figures come from HearingTracker's 2025 consumer survey, reported in 2026, which collected prices on 879 U.S. pair purchases. The average price paid was $3,432 per pair for prescription hearing aids bought privately without insurance — a figure that includes lower-priced Costco purchases — and $502 per pair for OTC hearing aids.
That is one survey of self-reported purchases, not a national statistic, and the OTC figure spans everything from basic amplifier-style devices to full-featured self-fitting aids. The structure behind it is the durable part:
- Prescription pricing is usually bundled. The price typically includes the exam, the fitting, verification, and a stretch of follow-up visits. You prepay for services whether or not you use them.
- OTC pricing is unbundled. You buy hardware plus app support. A professional exam, a real-ear verification, or an in-person adjustment is a separate purchase later.
Two costs people forget. First, the return window is the real price of an OTC experiment: a long trial makes a $500 device close to risk-free, while a two-week window with a restocking fee does not. Second, insurance rarely helps. Medicare states that Original Medicare "doesn't cover hearing aids or exams for fitting hearing aids" and that you pay all costs, though it notes Medicare Advantage plans may include extra hearing benefits, so check your plan. Fit, follow-up, and who fixes it when it is wrong
Assume the first setting will be wrong. What matters is how easily it gets fixed.
Physical fit is half the battle. OTC devices ship with a handful of silicone dome sizes. Too small and sound leaks out, producing whistling feedback; too large and the ear aches by dinner. Prescription devices can be built around a custom earmold, which matters more the more amplification you need.
Sound adjustment. OTC devices lean on an in-app hearing check and self-tuning controls, and better ones let a remote support agent push changes to the device. If you already dislike fiddling with apps, that is a genuine reason to pay for the clinic.
Who owns the problem. This is the honest core of the OTC hearing aids vs audiologist comparison. With OTC, the answer is you, backed by a support line and a return window. With a clinic, you book an appointment and someone re-measures, remakes the mold, and re-verifies against target inside the bundled period. Either way, get three things in writing first: the trial period, any restocking fee, and what repairs cost after the warranty ends.
Our view: the more comfortable you are troubleshooting your own gear, the more the prescription premium buys something you would not have used. The less comfortable you are, the more that premium is the product.
When OTC is the wrong choice
Some situations are not a value judgment. They are a signal to see a clinician first.
The FDA flags hearing loss or ringing in only one ear, or a noticeable difference between your two ears, as a red flag symptom, and advises seeing a doctor — preferably an ear, nose, and throat physician — when hearing loss is progressing or when there are associated symptoms such as dizziness, ear pain, or drainage from the ear canal. The agency requires OTC hearing aid labeling to carry these red-flag conditions on the package and in the instructions for exactly this reason.
Sudden hearing loss deserves its own line. NIDCD describes sudden sensorineural hearing loss — an unexplained, rapid loss of hearing all at once or over a few days — as something you "should consider a medical emergency and visit a doctor immediately," and warns that delaying diagnosis and treatment can reduce how well treatment works. NIDCD also notes that people often put off care because they assume the cause is allergies, a sinus infection, or earwax. Buying a hearing aid off a shelf in that window is the wrong move.
Severe and profound loss sits outside the category by law. The FDA is explicit that OTC hearing aids are not for treating severe or profound hearing loss. None of this is a diagnosis — it is the boundary the FDA drew. An audiologist or ENT can assess which side of it you are on, and that visit is worth its cost even if you ultimately buy over the counter.
How to decide
Work through these in order.
- Any red flags? Sudden change, one-sided loss, ear pain, drainage, vertigo, or ringing in one ear only — see a clinician before buying anything.
- How bad is it, honestly? The FDA's required OTC packaging language lists "you can't hear speech even if the room is quiet" as a sign to get help from a hearing health care professional rather than to self-treat.
- Do you want a number or a guess? A hearing test gives you an audiogram you own, which makes any later purchase — OTC included — better informed.
- Are you comfortable with an app? Self-fitting devices assume a phone, a quiet room, and patience. If that sounds unpleasant, weight the clinic higher.
- What is the return window? Do not buy an OTC device without a trial long enough to get through the awkward first two weeks.
- Is the listing honest? The FDA requires the words "OTC" and "hearing aid" on the outside of the box, along with the mild-to-moderate wording, the red-flag warnings, and the manufacturer's return policy. Treat "FDA-registered" or "FDA-certified" as marketing noise — the FDA says such statements may be misleading. A self-fitting device should have FDA clearance; simpler preset devices are exempt from that review.
- Budget check. Compare the OTC device against a bundled clinic quote, and ask the clinic what is included and for how long.
The bottom line
For an adult with a gradual, both-ears, mild to moderate hearing loss and no red-flag symptoms, OTC hearing aids are a defensible first purchase — and the best available randomized evidence found self-fitted devices performing comparably to audiologist-fitted ones at six weeks, with a follow-up of the same participants showing no meaningful difference at eight months. For severe loss, one-sided loss, sudden change, or any ear symptom, the clinic is not the premium option. It is the only appropriate one.
The trap is the middle: buying an OTC device for a loss it was never designed to handle, deciding hearing aids do not work, and giving up.
